So I am not sure it is just US Club skimming off the top of pyramid. However, directors at large youth clubs - who have to deal with USYSA (in any given state) bureaucracy day in and day out - some times reach a breaking point and are in the best position to make a move to US Club, especially if a group of them work in tandem. US Club reps reach out to them fairly regularly to see if they are still happy in USYSA world. Seems like there are parallels in the DA world as well (i.e. big clubs get tired of the top down mandates and have some bargaining power to go elsewhere).
Hear you on the promotion/relegation. The ABCs of leagues here has some destructive elements; the closed aspect of them makes it all the worse.
