Does anyone have experience where a player was moved from a B-Team to an A-Team? If so, what might you say led to the move and how did it play out? Is this possible at the older age groups? Is there a path?

I have seen one or two of these a year (my kid is 17) almost every year. Depends on the club (of course) and whether the A team is good or not. Players don't leave good A teams much unless they're moving up into a perceived better league so there aren't very many openings. In my experience there's actually more movement at the older ages as players "retire" once they realize they have other interests or make college commitments.

I've seen it several times and in two cases the elevated B team player has become one of the top five on the A team because they're surrounded by better talent.

"It depends." Sure it can happen. However typically the coach/club needs to be confident that B team player will be a meaningful contributor. Clubs don't typically swap out one interchangeable A team bench warmer for a top B teamer of similar ability (which is often the case in terms of skills). The other scenario is if an A team player leaves, but even then some clubs can and do bring in an outsider rather than bring in a B team players.
If a club is telling you it happens a lot - they're trying to sell you way too hard

Its all about the finances
if you are a B team player I am already cashing your check, so if I don't feel there is risk of losing you I am unlikely to offer you a spot that can bring me in another happy paying customer
Very few leave a club to join a B team for another so I need A team spits to attract more revenue

I have seen a few B team kids move to A, mostly around U12-13 as the rosters expand but also at U15 & 16 on ECNL teams. The key is that the B team player was very obviously the best on the RL team and clearly belonged on the ECNL squad and getting minutes. It does not happen often but if a kid is really good enough, they do not often get held back. Remember that there's a lot of injuries and changes that happen in HS years even on the best teams, so spots do occasionally open.
